Certificate of Attendance

For doctors needing to have a "Certificate of Attendance" for states outside of Texas and South Carolina. 

Each certificate is $5 per course; for example, if you took 4 courses and thereby need 4 certificates, then the total cost will be $20. 

On the next page, there will be a Registration Form that will have all the courses listed. 

Please CHECKMARK the courses that you need for the certificates and it will be sent to your email  within 2 weeks.
